Guidelines for Proof of Assets

Proof of Income	Personal Income Statement (for the past year)
	Withholding Tax Certificate (for the past year)
	Bank Deposit Balance Statement (for the past six months) Only bank-issued documents will be accepted; other formats will not be considered.
	National Health Insurance Enrollment Record (Issued within 3 months)
Proof of Property	Bank Deposit Balance Statement (Issued within 3 months) Only bank-issued documents will be accepted; other formats will not be considered.
	Property Ownership Certificate or National Property Tax Summary (Issued within 3 months)
	Insurance Policy Value Reserve Certificate (Issued within 3 months) Only accepts stamped and issued documents from the insurance company, and the Policyholder must be the applicant.
Proof of Investment	Investment Account Balance Statement (Issued within 3 months) The Holdings Statement exported via the 'Quick Functions' feature in the 'TDCC ePassbook' App.
